With breast implants inserted this way, the procedure involves the insertion of the saline implant through a 3 cm incision at the top of the armpit. The breast implant may be placed by the cosmetic surgeon just below the pectoralis muscle or above the muscle in the subglandular position.

Using an endoscope, i.e. a tube attached to video fiber-optic equipment that allows the breast augmentation with breast implants procedure to be visualized on a television screen, the pocket for the breast implant is carefully developed. Next the endoscope and saline tube are removed and the incision gets stitched up.

The scar (will be about 1 to 1-1/2 inches and in the folds of the belly button) is unlikely to be noticed unless the patient directly points it out.
